2025-2026 School Year – Renewal Applicants (open February 1, 2025 - June 30, 2026)

Step 1 – Submit the following documents through Google Forms here.

Select “Renewal Application” in form. You must have a google account to submit the required documentation.

  • Current (within 30 days) utility bill showing both the service address (top of bill) and billing address

- Make sure ALL pages are attached. Even if a blank page is shown, all pages must be submitted.

- If a utility bill is unavailable, these acceptable forms of address verification. 

- Limited Power of Attorney specifically authorizes Heritage Classical Academy (HCA) to endorse and deposit checks issued by the State of Ohio Educational Choice Scholarship Program (Ed Choice).

-All funds will be applied directly to your FACTS account.

- Print, sign, scan and upload this form. Wet signature is required; typed signature is not accepted.

Step 2 – Income Verification

If you completed regular income verification last year, no verification is needed. 

If you qualified for low income status, and wish to do so again, income verification is required. If not completed, you will qualify for the maximum award and be responsible for the difference in award and tuition.  

If you used the Minimum Scholarship Affidavit in 2024-2025, your minimum will be awarded. 

Income verifications do not come to Heritage Classical Academy but are used by the Department of Education to determine your award. 

Step 3 – Submit Acceptance Letter 

The State of Ohio will mail you a Scholarship Acceptance Letter once your scholarship is approved. Check the box to accept the scholarship, then sign and return the letter to HCA Finance Office at finance@heritageclassicalacademy.org
